Welcome to the Twin Lakes #4 School District
Welcome! We are so excited that your have decided to send your child(ren) to Twin Lakes #4 School District - Lakewood School. You may begin the registration process by clicking on the tab for New Families/Pre-registration for upcoming school year. Please fill out this Pre Registration Form.
Once your pre-registration form is completed you will be contacted for the next steps in the process. Please note, the following documents are required for new student registration. Please call the office to set up at time to submit them 262-877-2148.
Required Documents for new student registration - Three (3) Proofs of Residency are REQUIRED when moving into the district or if you have an address change.
- Child's Original Birth Certificate - no photocopies will be accepted.
- One (1) from this list:
- Lease, Rent Receipt, Mortgage Payment, or Property Tax Bill.
- Two (2) from this list:
- Driver's License/State ID, ONE utility bill, Insurance Policy, Bank Statement (Note: must be current/within last 30 days).
